Finances

Insurance, self-pay and what to ask before your visit.

We want you to understand the likely cost of a visit before you schedule. Our office can help check your plan and explain known charges, while your insurer makes the final decision about what it pays.

Insurance we currently accept

Rise Integrative Health is currently in network with Aetna and Horizon Blue Cross Blue Shield. What you pay still depends on your specific plan, the service, your deductible and copay, and any referral or authorization requirements. Contact us with your plan details before scheduling so we can help you check.

Insurance verification is an estimate of benefits, not a guarantee of payment.

UnitedHealthcare credentialing is pending. Until participation is confirmed, please ask the office about the current status and cost before you book.

Veteran assessment and nexus-letter fees

Some veteran services may have a flat fee. Before scheduling, ask what that fee covers, whether records review or follow-up is charged separately, and when payment is due. The fee covers clinical work and does not guarantee a nexus letter or a VA decision.

Cost estimates

You can ask for an estimate before you book. If the assessment shows that a different or additional service is needed, the final amount may change; we will explain any known charges and discuss the next step with you.

Cancellations and balances

We will share our payment and cancellation terms before your first appointment. Please read them and ask about any charge that is unclear before you sign.

What “in network” means for your visit

Rise Integrative Health currently lists Aetna and Horizon Blue Cross Blue Shield as in network. UnitedHealthcare participation is pending. Your exact plan and the service you need still matter: a covered office visit does not tell you what a separate assessment, records review or written opinion will cost. Ask both the office and your insurer to check the physician, visit type, deductible, copay, coinsurance, and any referral or authorization requirements before care begins.

Self-pay and out-of-network care

If you plan to pay directly, ask the office for the current written fee for the initial visit, follow-up, evaluation, records review or written opinion you need. We can tell you whether a superbill is available if you plan to seek out-of-network reimbursement. Your insurer decides whether it will reimburse any part of the charge.

Questions to ask before scheduling

  • Is Dr. Khadtale in network for my exact plan?
  • Is this service covered, and do I need a referral or prior authorization?
  • What are my deductible, copay, coinsurance and telehealth benefits?
  • Will an assessment, records review or paperwork have a separate charge?
  • If I am paying directly, what is included in the quoted fee and when is payment due?

Before your visit

How to check your cost

01Share your plan

Tell us what kind of visit you need. Ask the office how to share your plan name and member ID securely.

02Check network status

Ask about participation and the current self-pay fee if relevant.

03Review your benefits

Confirm your deductible, copay and telehealth rules directly with your insurer.

04Review the estimate

Read the estimate and office payment and cancellation policies before scheduling.

Questions about cost?

Call or email with a brief question about current fees and coverage. Ask the office how to share your member ID or medical details securely.

Please note: Network participation does not guarantee coverage. Benefits, deductibles, copays, prior authorization requirements, and final claim decisions are determined by the patient’s plan and insurer.
