Start with the purpose of the assessment

Explain what brought you in, when it began and how it affects work, home or relationships. Bring a current medication list, including nonprescription products and supplements, and tell the clinician about previous treatment or side effects.

Ask what the information suggests and what is still uncertain. At Rise, the next step may be treatment, more information, follow-up or a referral, depending on the assessment.

Discuss medication as a shared decision

If medication is considered, ask what it is intended to help, what benefits and risks to expect and how the plan will be reviewed. People can respond differently to mental health medications, so an individual plan may need adjustment with a clinician.

Tell your clinician about everything you take so possible interactions can be considered. Do not stop or change a prescribed mental health medication without discussing it with your healthcare provider.

Make follow-up more than a refill

Bring examples of what has improved, what remains difficult and any new concerns. Note side effects and changes in daily functioning. A brief list is often easier to discuss than trying to remember everything during the visit.

Before you leave, ask when to return and how to contact the care team between appointments. Ask which symptoms or side effects need prompt attention.

Questions to bring

  • What does the assessment support, and what needs clarification?
  • What are my options and the reasons for each?
  • How will we judge whether treatment is helping?
  • Which side effects should I report, and how?
  • When is the next review, and who coordinates other care?

Write down the next step in your own words. If it is unclear, ask for an explanation before ending the visit.
